PHCA Mask Policy

  • Masking will be based on community transmission levels in our area.
  • Signs are posted on clinic doors and in exam rooms noting whether levels are HIGH or NOT HIGH,


COVID-19 Vaccine

  • The vaccine is available at previously scheduled appointments (such as a wellness visit) or patients can schedule an appointment specifically to receive the vaccine. 


COVID-19 Testing

PHCA offers in-office rapid testing for results while you are on-site, which will be billed to insurance.

Prompt pay fees for self-pay patients:

  • Rapid test for COVID-19: $46 
  • Combination rapid test for COVID-19 and flu: $75

We also offer PCR testing which is sent to an outside lab. Billing and payments for this service are handled by the outside lab.


Online Curbside Check-In

PHCA now offers online curbside check-in. Patients remain in their cars upon arrival, and check-in using a link sent by email or text. Then our staff will text or call to meet you at the front door and escort you directly to your exam room.

Mobile check-in is being used for ALL patient visits at all locations, including evening and weekend hours.

PHCA offers telemedicine for select visit types, where patients remain at home and connect with a provider online in a HIPAA-compliant video conference. Telemedicine is available from all PHCA offices for weekday appointments. During scheduling calls, our staff will determine if your visit needs are eligible for telemedicine and let you know what to expect for a telemedicine appointment.
