2014 Medicaid/MediPass Changes

Update 5/29/14 - Patients should not select Amerigroup Healthy Kids, that is a different plan we do not accept. We are only accepting the plan titled "Amerigroup".

Important Notice: MMA Plan Enrollment/Change Deadline

Dear Parents,

As of June 1, Medicaid has implemented the Florida Managed Medical Assistance Program (MMA). This program offers a number of managed care plans to Medicaid recipients, and you should have selected one of these plans by June 1 if you were notified by Medicaid.

If you did not select a plan on your own, you may have been automatically enrolled on a plan after the deadline passed. This letter is to help you understand how to change your plan if needed, so you can stay with your PHCA pediatrician and office.

If you were automatically enrolled in a plan, please verify it is one of the following plans. These are the only MMA plans we are accepting:

Please make sure you are currently enrolled on one of these plans, or change your enrollment to one of these plans in order to stay with your current Pediatric Health Care Alliance pediatrician and office. You should select one of these plans even if you do not see your pediatrician’s name listed as a participant.

Important Notes:

  • Recipients who were automatically enrolled in a plan have a limited window to change plans.

  • Plan changes must be completed by September 1, 2014.

  • It is imperative to verify your plan enrollment with AMERIGROUP or SUNSHINE HEALTH, or change to one of these plans so you can stay with our practice.

  • After September 1, if you are not enrolled in AMERIGROUP or SUNSHINE HEALTH, we will not be able to see your child at PHCA.

  • Our offices will continue to accept Medicaid for the limited number of patients who are allowed to stay on Medicaid rather than select a MMA plan (you will be notified by the state if this applies to you).

If you have questions about updating your records with new health plan information, or about the health plans we accept, please contact your pediatrician’s office.

Thank You,
Karalee Kulek-Luzey, M.D.
