Available Positions: Case Manager  |   Certified Medical Assistant   |   Licensed Practical Nurse   |   Patient Service Representative

Case Manager

Current Openings/Locations (PT or FT; min 20 hrs/wk): Oldsmar, FL

The LPN case manager has the overall responsibility for providing and coordinating patient screening, interventions and ongoing care management for the pediatric population. Uses nursing knowledge and the nursing process to provide advocacy, assessment, planning, communication, education, resource management and service coordination. 

Key Responsibilities

  • Conduct telephonic outreach to patients identified with recent hospitalizations, identified as high risk and high cost, having frequent emergency room visits or chronic conditions.
  • Screening and assessment of patients to promote successful transitions of care, management of chronic conditions, and need for resources and education. Uses clinical judgement to assess and evaluate patients at risk for repeat hospitalization and ER visits.
  • Establish a plan of care to provide education, resources and support to ensure adequate management of their condition and access to health care services.
  • Provide clear communication with the patient and provider. Communication of patient status and plan of care with provider as needed.

Minimum Qualifications for this Position Include:

  • Education – Graduate of an accredited School of Nursing
  • Job Experience – One (1) year of pediatric experience in a clinical setting.
    • Knowledgeable in Case Management principles, procedures, and practices.
    • Knowledgeable in clinical quality indicators (HEDIS, Care Gaps).
  • Professional License/Certification – Current Florida Licensed Practical Nurse

Certified Medical Assistant

Certified Medical Assistants (CMAs) are accountable for performing patient care activities associated with the personal needs and comfort of patients served by the practice, assisting medical providers and maintaining a clean and safe environment in accordance with the policies and procedures of the practice. Responsibilities include but are not limited to;  obtaining vitals, administering injections and immunizations, assisting with triaging, urinalysis, well visits, rooming patients and other tasks as assigned.

Minimum qualifications for this position include:

• A High School diploma or equivalent.
• Prior medical experience - preferably in pediatrics.
• Computer knowledge including the ability to type and knowledge of Microsoft Office, Microsoft Windows, and Electronic Medical Records (EMR).
• A strong focus on customer service and quality outcomes with the ability to work with patients, doctors, patient care staff, and families to provide optimal levels of patient care.
• Knowledge of current medical office procedures.
• Ability to read, understand and follow oral and written instruction, and the ability to speak clearly and concisely.
• A nationally accredited certification or the ability to obtain a certification within 90 days of hire.

Licensed Practical Nurse

Licensed  Practical  Nurse  (LPN)  is  accountable  for  performing  nursing  care activities associated with the personal needs and comfort of patients served by the  practice,  assisting  medical  providers  and  maintaining  a  clean  and  safe environment in accordance with the policies and procedures of the practice. Responsibilities include but are not limited to;  obtaining vitals, administering injections and immunizations, assisting with triaging, urinalysis, well visits, rooming patients and other tasks as assigned.

Minimum qualifications for this position include:

• A High School diploma or equivalent. Graduate from an accredited nursing school.
• Prior medical experience - preferably in pediatrics.
• Computer knowledge including the ability to type and knowledge of Microsoft Office, Microsoft Windows, and Electronic Medical Records (EMR)
• A strong focus on customer service and quality outcomes with the ability to work with patients, doctors, patient care staff, and families to provide optimal levels of patient care.

• A current state of Florida nursing license.

Patient Service Representative

Current Openings/Locations (FT/day): Crossroads Office; FishHawk Office

Patient Service Representatives (PSRs) greet, instruct, direct and schedule patients and visitors.  Other responsibilities include processing correspondence, answering telephones, maintaining files, verifying demographic information and insurance coverage, collecting co-payments, reviewing patient accounts and promoting patient satisfaction at all times.

Minimum qualifications for this position include:

• A High School diploma or equivalent. Coursework in medical terminology a plus.
• Prior medical front office experience - preferably in pediatrics.
• Computer knowledge including the ability to type and knowledge of Microsoft Office and Microsoft Windows, and Electronic Medical Records (EMR).
• A strong focus on customer service and quality outcomes with the ability to work with patients, doctors, patient care staff and families to provide optimal levels of patient care.
• Knowledge of current medical office procedures including an understanding of medical terminology and familiarity with insurance.
• Knowledge of grammar, spelling and punctuation to type patient information.
