Case Management Nurse (Oldsmar, FL)

This is an exciting opportunity to join our forward-thinking team and support our innovative approach to pediatric population health management and initiatives to improve the quality of care for patients.The Case Management Nurse coordinates the team approach to management of patient care on an in-patient as well as an outpatient basis. This position works closely with the Medical Director to coordinate the care of at risk patients by educating and providing assistance to patients regarding health care resources and communicating with primary care physicians.

Other duties include identifying and managing patients who are deemed to be high-risk using EHR reporting, billing data & communication with physicians and clinical staff, assisting high-risk patients with arranging follow up visits with primary care physicians and referrals to specialist and diagnostic procedures, informing and educating families of high-risk patients about available community resources, appropriateness of care and basic disease management and documenting and communicating patient outreach in individual patient charts and with the patients primary care physician.

Job Requirements:

  • Education: Registered Nurse or LPN required.  Bachelor’s degree preferred.
  • Experience: 3 or more years in a managed care setting and/or recent clinical experience. Experience and understanding of the confidentiality of medical records. A thorough understanding of the managed care environment from the perspective of the member, physician and hospital. 
  • Knowledge, Skills & Abilities: Ability to prioritize and organize work area despite multi-faceted task directions and interruptions. Ability to handle difficult people/situations with tact and diplomacy. Excellent interpersonal skills with the ability to interface with physicians, hospital staff, members and health plan representatives in a tactful, informed and customer-oriented manner. Strong written and verbal communication skills. Intermediate to advanced MS Applications skills including MS Word, Excel, Access, Outlook and PowerPoint a plus.
  • Certification/Licensure: Must have current registration in nursing. Certified Case Manager (CCM) certification preferred.

Certified Medical Assistant (multiple locations)

Certified Medical Assistants (CMAs) are accountable for performing patient care activities associated with the personal needs and comfort of patients served by the practice, assisting medical providers and maintaining a clean and safe environment in accordance with the policies and procedures of the practice. Responsibilities include but are not limited to;  obtaining vitals, administering injections and immunizations, assisting with triaging, urinalysis, well visits, rooming patients and other tasks as assigned.

Minimum qualifications for this position include:

• A High School diploma or equivalent.
• Prior medical experience - preferably in pediatrics.
• Computer knowledge including the ability to type and knowledge of Microsoft Office, Microsoft Windows, and Electronic Medical Records (EMR).
• A strong focus on customer service and quality outcomes with the ability to work with patients, doctors, patient care staff, and families to provide optimal levels of patient care.
• Knowledge of current medical office procedures.
• Ability to read, understand and follow oral and written instruction, and the ability to speak clearly and consicely.
• A nationally accredited certification or the ability to obtain a certification within 90 days of hire.

Licensed Practical Nurse (multiple locations)  

A  Licensed  Practical  Nurse  (LPN)  is  accountable  for  performing  nursing  care activities associated with the personal needs and comfort of patients served by the  practice,  assisting  medical  providers  and  maintaining  a  clean  and  safe environment in accordance with the policies and procedures of the practice. Responsibilities include but are not limited to;  obtaining vitals, administering injections and immunizations, assisting with triaging, urinalysis, well visits, rooming patients and other tasks as assigned.

Minimum qualifications for this position include:

• A High School diploma or equivalent. Graduate from an accredited nursing school.
• Prior medical experience - preferably in pediatrics.
• Computer knowledge including the ability to type and knowledge of Microsoft Office, Microsoft Windows, and Electronic Medical Records (EMR)
• A strong focus on customer service and quality outcomes with the ability to work with patients, doctors, patient care staff, and families to provide optimal levels of patient care.
• Knowledge of current medical office procedures.
• Ability to read, understand and follow oral and written instruction, and the ability to speak clearly and consicely.
• A current state of Florida nursing license.

Patient Service Representative (multiple locations)

Patient Service Representatives (PSRs) greet, instruct, direct and schedule patients and visitors.  Other responsibilities include processing correspondence, answering telephones, maintaining files, verifying demographic information and insurance coverage, collecting co-payments, reviewing patient accounts and promoting patient satisfaction at all times.

Minimum qualifications for this position include:

• A High School diploma or equivalent. Coursework in medical terminology a plus.
• Prior medical front office experience - preferably in pediatrics.
• Computer knowledge including the ability to type and knowledge of Microsoft Office and Microsoft Windows, and Electronic Medical Records (EMR).
• A strong focus on customer service and quality outcomes with the ability to work with patients, doctors, patient care staff and families to provide optimal levels of patient care.
• Knowledge of current medical office procedures including an understanding of medical terminology and familiarity with insurance.
• Knowledge of grammar, spelling and punctuation to type patient information.
